Retrograde Ejaculation: Causes, Symptoms & Fertility Treatment

Retrograde ejaculation is when semen travels backward into the bladder during orgasm instead of exiting through the urethra — the orgasm itself still happens, but with little or no visible ejaculate. It's one of the more confusing fertility issues a man can run into, because everything feels normal except the one thing that's supposed to happen doesn't. It's also more common than most men realize, and in many cases, treatable or workable around.

What It Feels Like

• A "dry orgasm" — climax with little to no semen released
• Cloudy urine after sex, since semen mixed with urine is what actually happens instead
• Reduced fertility or difficulty conceiving, often the reason it gets diagnosed in the first place
• No pain and no change in the sensation of orgasm itself for most men — this is what throws people off, since it doesn't feel like anything is wrong

What Causes It

The bladder neck is supposed to close during ejaculation, directing semen forward. When that mechanism doesn't work properly, semen takes the path of least resistance — backward into the bladder. Common causes include:

• Diabetes — nerve damage (diabetic neuropathy) is one of the most common underlying causes
• Prostate or bladder surgery, including transurethral resection of the prostate (TURP) and bladder neck procedures
• Certain medications — alpha-blockers (often prescribed for prostate symptoms) and some antidepressants are frequent culprits
• Spinal cord injury or multiple sclerosis, which can affect the nerve signals controlling the bladder neck

Identifying which of these applies to you matters, because the cause largely determines whether it's reversible.

How We Diagnose It

Diagnosis is usually straightforward: a post-ejaculation urinalysis checks for the presence of sperm in the urine, which confirms semen is going into the bladder rather than being lost or simply not produced. This distinction matters — retrograde ejaculation is a very different problem from azoospermia, even though both can show up as a semen analysis reading with little or no visible ejaculate.

If Medication Is the Cause

If a medication you're taking (commonly an alpha-blocker or certain antidepressants) is the trigger, switching or adjusting that medication — in coordination with the prescribing doctor — often resolves it entirely.

Medication to Help the Bladder Neck Close

For cases not caused by surgery or significant nerve damage, certain medications can help the bladder neck close more effectively during ejaculation, redirecting semen forward again. This doesn't work for everyone, particularly after certain surgical procedures where the anatomy itself has changed, but it's often worth trying first since it's the least invasive option.

Sperm Retrieval From Urine, for Fertility

If you're trying to conceive and the underlying cause isn't reversible, sperm can often be retrieved from a urine sample collected shortly after ejaculation (the bladder is alkalinized beforehand to protect sperm viability) and used for IUI or IVF/ICSI. This means retrograde ejaculation, even when it can't be reversed, doesn't have to mean the end of the road for biological fatherhood.
